Hello,Every year as part of our commitment to continuous quality improvements, Community Living BC (CLBC) conducts an Annual Service Satisfaction Survey by telephone with a randomly selected group of self advocates and family members from across the province.
The results from this survey help inform us about areas of service delivery that may need improvement and where supports and services are working well. The information also guides us in the development of our service and strategic plans. This year, the survey will be conducted between December 1, 2014 – December 21, 2014 by Mustel Group, an independent and well-respected research firm. For the purposes of this survey, CLBC is providing Mustel with a list of individuals and family members who are receiving CLBC supports. Mustel Group uses this list only while completing the survey and will delete it once the survey is completed. We value the input we receive from the people we serve, and thank you in advance for your participation if you are contacted for this brief survey. All responses to the survey questions will be completely confidential and anonymous, and no identifying information is provided to CLBC with the results. If you are contacted, Mustel will first ask you if you would like to participate. You can choose to say “no” if you are not comfortable with participating.
If you have any questions or need further information on the survey, you can contact Jule Hopkins, Manager Policy and Program Initiatives by email at jule.d.hopkins@gov.bc.ca or by phone at 604-664-0167.
